In order for us to expedite your request for repair service of your CTX product, please complete the following form in its entirety. Also a copy of original proof of purchase or invoice MUST be faxed to the above fax number together with an ID number which will be given to you after you complete
this form.
Be advised that you will receive a RMA number and complete instructions
via e-mail once both items are received by
CTX RMA Dept. Please allow 72 hours (exclude holidays and weekends)
before receiving your RMA number. Thank you for your
time and cooperation.
RMA Product Requirements:
CTX will not accept any return product for repair without a valid RMA number. RMA number must match serial number. RMA number expires
30 business days after being issued. If customer doesn't returns the unit within
30 business days, Please call our Technical
Support before processing a new RMA number.
Due to necessity of testing repaired/replaced unit(s), please expect at least
10 working days prior to pick up/shipment of the repaired/replaced unit(s).
